General
Japan have won the most gold medals in the women's 4x100m freestyle relay at the Asian Games with 10, three more than their closest rivals the People's Republic of China. However, the Japanese team had to settle for silver behind the Chinese team at five consecutive Games between 1998 and 2014. Of Japan's 10 gold medals, nine came between 1954 and 1994.

The Japanese team returned to the top of the podium in 2018 in Indonesia. The People's Republic of China had recorded the fastest time in the heats, but Japan triumphed one place ahead of their Chinese rivals in the final. The team have six silver medals to add to their 10 golds, meaning they have won either gold or silver at every edition of the Asian Games to feature the event except one [1990].

After reaching the Olympic final at two Games in a row in 2012 and 2016, Japan failed to make the final when they hosted the 2020 Games in Tokyo, swimming the ninth-fastest time in the heats. They qualified for the world championship final in 2017, 2019 and 2023, most recently ranking eighth in home water at the 2023 World Championships in Fukuoka.

At the Asian championships, Japan won silver in both 2012 and 2016 and bronze in 2009.
