General
Japan have been the dominant force at the Asian Games in the men's 4x200m freestyle relay. They have won 16 of the 17 gold medals on offer, with the only blemish on their record coming at Guangzhou 2010, where hosts the People's Republic of China claimed gold and forced Japan to settle for silver.

Japan's total of seven Olympic medals in the event is bettered only by the United States of America [24] and Australia [nine]. They were Olympic champions in back-to-back Games in 1932 and 1936, won silver medals in 1928, 1952 and 1960, and claimed bronze in 1964 and 2016. The Japanese team's best result at the world championships is a fourth-place finish in 2009 in Rome. They failed to reach the world championship final, ranking ninth, on their last two appearances in 2019 and 2023.

At the Asian championships, Japan won gold in 2016 and silver in both 2009 and 2012.
